- You must be an Avril Lavigne fan
- You don't have to have an Avril site, but if you do please take the time to add a code or banner to your site (must run off of your own server, direct linking will use up my bandwidth and i will have to delete you)
- Please fill in the whole form properly and sensibly
- In the rules section where it says "Is Avril The Best?" be sure to fill in "You Bet" so I know you have bothered to read these rules
